Students in their Senior and Junior years of High School who receive Special education services, enrolled in the 504 plan, have a physical disability, or mental health issue may be eligible for services through their high school and Vocational Rehabilitation. Eligibility is determined by the IEP team along with the Vocational Rehabilitation Counselor.
The Team conducts observations of the student at school and work sites; interviews teachers, parents, and those supporting the student. This is a student-centered program with emphasis on the individual students skills and accommodations.
Students in this program first and utmost are encouraged to complete high school. The assessment is a tool to assist students and parents to use the assessment with recommendations to make decisions on post high schools plans.
Assessments include the evaluation of a variety of abilities such as appearance, behavior, strength and endurance, speed and judgment. Reports are complied based upon work samples, classroom observations, teacher, student, and parent interviews. The report will include topics such as plans for future living arrangements and post-secondary activities.
Schools must have co-op agreement with their Vocational Rehabilitation Office and a Memorandum of Understanding (MOU) with Choices for People Center to be eligible for High School Services.
