ALL SERVICES ARE FUNDED THROUGH ROLLA REGIONAL CENTER/DEPT. OF MENTAL HEALTH OR PRIVARTE PAY FROM THE INDIVIDUAL LIVING IN THE COMMUNITIES OF PHELPS COUNTY.
INDEPENDENT SUPPORTED LIVING (ISL): Choices for People Center s Independent Supported Living program offers a variety of services and supports to individuals who live in their own homes in the community or within an ISL. Levels of supports for these individuals range from 24 hr. staffing in the individualized supported homes, to support for individuals who need assistance for a few hours each week. The types of services and supports for the home and the community may include: instruction in, or assistance with, daily living skills, home care, personal/home/community safety, personal grooming, health/medical issues, social interaction, financial management, leisure time/recreation, and exercise. The CP Center provides 24-hour emergency response and protective oversight for the individuals in this program and can act as a social security benefit payee. Transportation is provided as funding allows.
HOME MANAGEMENT: Choices for People Center provide supports to individuals who live within their own homes. Staff assists and oversees the individuals with bill paying, Dr. appointments, up keep of their home, medications, grocery shopping, and any other basic daily needs. Transportation is provided as funding allows. The Choices for People Center may also be the payee of these individuals.
COMMUNITY INTEGRATION: This program works with individuals who reside within their own home or within a RFC. Choices for People Center staff support the individual learning or utilizing their social skills while participating in community activities. Some of these activities may include shopping, socialization, volunteering, swimming, fishing, basketball, painting, and Special Olympic events.
